Google Ads for Clinics and Medspas: What to Spend, What to Track, What to Avoid

Jason Byer -- AI ConsultantSeptember 8, 20261 min read

Most practices that "tried Google Ads and it didn't work" had the same problem: the ads worked, but everything after the click didn't. The phone rang out, forms sat unread until Monday, and the account was optimised for clicks instead of booked appointments.

Why paid search still matters

SEO and the map pack are the long game. Search ads are the short game: you can be at the top of "Botox [city]" this week. And healthcare searches convert well.

How to structure the account

  • Campaigns by service, not practice. Run separate campaigns for services like neuromodulators, laser hair removal and new family-medicine patients, each with its own budget, keywords and landing page. One catch-all "Clinic – General" campaign is how budgets disappear. Separate campaigns show you which services pay back.
  • Match keywords to intent.
  • High intent: "[service] near me," "[service] [city]," "book [service]," "same day [specialty]." Put most of your budget here.
  • Research intent: "how long does Botox last," "what is a DEXA scan." These clicks are cheaper but convert less, so content and SEO handle them better.
  • Negatives from day one: "jobs," "salary," "training," "course," "free," "DIY," and competitor staff names. Review the search terms report weekly for the first month.
  • Send each ad to a page built to book: Don't send ads to your homepage. Use a matching page with reviews, pricing or insurance info, and one-step booking.
  • Track bookings, not clicks. Import calls over 60 seconds, booked appointments and show-ups so Smart Bidding learns what a real patient looks like.

Policy and privacy rules

Remarketing is limited. Google classes health as a sensitive interest category, including cosmetic surgery and injections. You can't use your own remarketing or Customer Match lists for these services, so build campaigns around keywords, location and schedule. Some categories also need certification before ads will run, such as addiction services, telemedicine prescribing and prescription drugs (sometimes via LegitScript).

Tracking pixels and HIPAA. HHS OCR's 2022 bulletin warned that tracking tech can disclose PHI (personal health information) to ad platforms. In June 2024, AHA v. Becerra struck down the part of that guidance covering IP addresses on public, unauthenticated pages. But pixels on portals, booking flows or intake forms still need review and a BAA where PHI is involved.

Where budgets actually leak: the response

A Harvard Business Review study of 1.25 million leads found that companies responding within an hour were nearly 7x more likely to qualify the lead. A patient who clicks your ad at 7:40pm is also clicking two of your competitors.

  1. Answer every call. A [voice AI receptionist](https://www.clinicdigital.co/offers/voice-ai) covers after-hours and peak times. See [voice AI vs. an answering service](https://www.clinicdigital.co/blog/voice-ai-vs-answering-service).
  2. Respond to forms in minutes. Send an instant text with a booking link.
  3. Book on the landing page. A chatbot or online scheduler turns a 10pm click into an appointment.

That full loop is what our Patient Acquisition Engine is built for.

What to measure monthly

  • Cost per booked appointment by campaign
  • Lead-to-booking rate and median response time
  • Missed-call rate during ad hours
  • Show rate for ad-sourced patients
  • Search terms that spent money without converting

If you track only one number, make it cost per booked appointment. It holds the ads, the front desk and the follow-up to one standard, the same way patients experience them.
